Clinical Management of Acne - Overview
General Topics have looked at the key causes of acne and spots and determined the best clinical methodology for effective acne treatment The Synchroline range of products combined with Enerpeel® peels can control the symptoms and underlying causes and also manage a number of post acne consequences such as acne scarring and pigmentation effectively. Individual clients should be assessed as to which products they will need. Here we describe the various acne treatment systems and list them in an acne management protocol at the end of this section. Detailed individual acne product information is contained in these training notes and elsewhere.
We have devised an acne protocol table which gives an at-a-glance view of all the products in the SkinMed acne protocol. Click here to see it..

AKNICARE®
The Medical Device For The Treatment of Acne, Spots and Oily Skin

The two key medical devices to control acne effectively - Aknicare® Lotion and Aknicare® Cream change your skin from within and achieve the six effects listed above. Also Aknicare® provides two types of preparatory products - Aknicare® Cleanser - an antibacterial exfoliating wash for those who need to reduce surface debris and sebum (oil) and Aknicare® Remover - an active cleanser with non exfoliating non irritating cleansing agents for angry skin with often inflamed spots.
The Remover also contains an anti-inflammatory and tri-ethyl citrate which like retinol normalises skin turnover. This may be more suitable for those with angry skin or those who are having an Enerpeel® Salicylic Acid peel which may be exfoliating enough. The Aknicare® range is completed by also providing active sebum reducing camouflage make-ups and primers called Aknicare® Fluid Powders and also Aknicare® Skin Roller – another medical device for reducing individual spots and ‘zits’, targeting individual blemishes rapidly.

ENERPEEL®
For the management of acute and chronic acne both comodonal and inflammatory acne and also acne scarring

Enerpeel® is the revolutionary new chemical remodelling system which delivers key active ingredients into the skin more efficiently than traditional peeling solutions. Incredibly we also incorporate key active ingredients in with the peels to further control the specific skin condition itself. For acne we offer several solutions which are used in conjunction with Aknicare®.
Enerpeel® SA is a skin remodelling solution designed specifically for sufferers of acne, thanks partly due to the sebostatic (oil production reducing) effect of Salicylic Acid. The solution also contains the two active ingredients that are present in Aknicare® - triethyl citrate and ehtyl linoleate - which were shown in a clinical trial by Professor Tony Chu to reduce the four main visible signs of acne.
Enerpeel® PA is a version of the frequently used pyruvic acid chemical peel in a patented Enerpeel® skin remodelling solution. It is used as part of an acne protocol to reduce an superficial acne scarring as well as oily skin. As it is contained in an Enerpeel® solution, any redness and erythema is much less in severity and duration, compared to many other chemical peels.
Based upon salicylic acid, lactic acid and resorcinol, Enerpeel® JR is used primarily to treat areas of hyperpigmentation following acne. It also has a remarkable effect in reducing keratin build up after exfoliation. After treatment, there will be a marked reduction in visible acne lesions, with pigmentation suppression and reduction It can also be used as a preparation peel for medium to deep peeling as seen with both strengths of Enerpeel® TCA.
Enerpeel® TCA - the most effective skin remodelling solution in the Enerpeel® range - is available in two different versions 25% (which is used to treat visible acne scarring) and 40% (which is used to treat deep scars, even ice pick scars). It will also help to remove superficial discoloration, similar to that found in post acne pigmentation.
Enerpeel® Hands (TCA/LA) is a chemical solution used to combat more visible acne scarring in those people at pigmentation risk (Fitzpatrick 4 to 6). Containing a highly effective remodelling agent known as Trichloroacetic Acid as as well two acids (Lactic Acid and Kojic Acid) which are renowned for their ability to reduce excessive pigmentation, Enerpeel® Hands is truly a revolutionary peel.

SYNCHROVIT®
Retinol A Creams, which Increase Targeting of Cellular Turnover

We have also incorporated Synchrovit® Retinol A products. Aknicare® contains triethyl citrate which normalises cell turnover and reduces skin thickening and keratinisation. For those who wish to use retinol as well for this purpose, Synchrovit® provides a 1% strength retinol in conjunction with tocopherol or Vitamin E. These two ingredients synergise and give a better overall effect than Retinol A alone.

TERPROLINE® & FILLAST®
Skin Healing, Recovery and Scar Prevention

Terproline® Professional Creams provides the raw materials for making more elastin (fibronectine) and collagen while also stimulating the fibroblasts, the cells in the skin that convert these raw materials into collagen and elastin. This improves skin structure and can be used to heal and whiten acne scars. This is the professional range clinically-trialled in burns units to reduce burn scars and also used by plastic surgeons to reduce operation scars.
It is also used by laser resurfacing clinics to reduce erythema and improve healing times from 10 to 3 weeks following full facial resurfacing. It can also be used to improve the results and reduce the complications of any treatment that challenges the skins integrity such as physical peeling, light and laser treatments and also injections such as Botox and Fillers.
Terproline® should always be used in conjunction with Fillast®, which supplements and encourages manufacture of hyaluronic acid. Hyaluronic acid aids the repair of skin tissue following acne scarring and also helps protect, develop and maintain the collagen which Terproline® encourages.

THIOSPOT®
To Manage Post-Acne Pigmentation Complications

After spots have healed particularly in darker skin types, one can be left with dark pigmented marks. This is due to the damage caused by the spot and/or picking the spot which causes increased pigment production in the area. This is a protective response from the skin. Use Thiospot® Skin Roller – a non-hydroquinone, non-arbutin, non-bearberry effective way of normalising pigmentation so that marks disappear over 4 to 8 weeks. Thiospot® has been clinically proven to be as effective as 2% hydroquinone cream but without the associated cancer stigma.

HYDRATIME® & NUTRITIME®
Lipid Boosting and Water Binding

Long term acne sufferers may develop a chronic lipid deficiency and enhanced lipid boosting may be required before full acne control can finally be achieved. Hydratime® and Nutritime® supply the whole range of ceramides and ceramide precursors which promote better lipid levels. They also supply a number of water binding agents like hyaluronic acid and saccharide isomerate and others to boost hydration generally and slow down over active trans-epidermal water loss TEWL, another key feature of acne skins. Lipid boosting has lead to final resolution in some chronic sufferers.
