Every year, millions of dollars, pounds, euros and even yen are spent on anti-aging products in the unwavering quest to look younger. But treating wrinkles and fine lines often requires more than is available over the cosmetics counter. Skin fillers have become very popular for treating facial wrinkles and for combating aging skin. For years, dermatologists have known that wrinkles result from the loss of three crucial skin components – collagen, elastin and hyaluronic acid.
The First Fillers
While bovine collagen has been the undisputed filler of choice for the treatment of facial wrinkles since injectable skin fillers were introduced in the 1970s, its short-lived results and potential for allergic reactions limited its potential. More recently, human bioengineered collagen – which consists of collagen derived from human cells – was approved for treating facial wrinkles, acne scarring and lip reshaping. Although human bioengineered collagen is an improvement over bovine collagen because it does not pose an allergy risk, dermatologists still sought a filler that would safely and effectively replace hyaluronic acid, the other primary component lost in aging skin.
So new fillers emerged that are able to replace the naturally-occurring sugars in the skin known as hyaluronic acid; these fillers work by pulling water into the skin, resulting in increased skin plumping and volume.
Hyaluronic Acid Fillers
Hyaluronic acid is a naturally-occurring component of the skin that functions by holding together collagen and elastin, providing a framework for the skin. When injected into the skin in gel form, hyaluronic acid binds to water and provides volume to easily fill in larger folds of skin around the mouth and cheeks. Patients notice an immediate plumping of the skin in the treated areas.
One of the main advantages of hyaluronic acid gel is that it does not pose an allergy risk for patients and there is no risk of transmitting animal diseases by injection. Since a skin check for allergies is not required with hyaluronic acid gel, patients can be treated on their first visit to the dermatologist. The results of hyaluronic acid injection last approximately four to six months and require less volume to fill wrinkles and hard-to-treat skin folds compared to collagen.
Despite its numerous benefits hyaluronic acid gel does not contain an anaesthetic, so injections can be painful. In addition, there is usually temporary inflammation that produces swelling and redness following injection with hyaluronic acid gel – especially in the lip area.
The Secret way to stay looking young
It is known that up until our late twenties, give or take a few years, the cells called fibroblasts are very active in producing collagen, fibronectine (elastin) and hyaluronic acid. The collagen we make is called Type III collagen and is made of short fibres and is very ‘fluid’ and gives us a softer face shape.
Secondly our fibroblasts make hyaluronic acid which is a long chain repeating units of two sugars called glucuronic acid and acetyl glucosamine. Each of these molecules can capture and hold up to 400 molecules of water and becomes a jelly like filling substance which plumps out the skin and keeps our skin hydrated and wrinkle free.
Our fibroblasts also make fibronectine, known as elastin, which are fibres made from glycoproteins which give our skin elasticity so as we stretch the skin through expressions etc it gives the ability to elastically return to its normal shape.
The relationship between these three fibres creates the structural framework of the skin in which all our skin cells sit.
As we reach maturity our fibroblasts start to slow down and we make less of these fibres. As hyaluronic acid is broken down by free radicals and an enzyme in the skin called hyaluronidase then our total hyaluronic acid levels begin to drop. This means the skin cannot hold so much water and we start to develop lines and wrinkles. However there is another and maybe greater effect as a result of this.
The drop in hyaluronic acid triggers a change in our collagen. The short fluid collagen Type III starts to bind together into longer more rigid fibres of collagen known as collagen Type I. This gives our face a more angular, rigid shape or an older face shape.
So we can use hyaluronic acid fillers to reduce wrinkles and fine lines by replenishing levels but our collagen has already changed and given us an older face shape albeit wrinkle free. So what is the answer? You could inject hyaluronic acid into the dermal layers in your late twenties to maintain collagen Type III and some do but this is not for most people, they will not have injections until they see the signs of aging.
You could take hyaluronic acid supplements or drinks but hyaluronic acid is a very big molecule and will not reach the skin, more likely you will end up with smooth intestinal wall. You cannot put hyaluronic acid serums and creams on the skin as again, because hyaluronic acid is a big molecule you will only get it into the epidermal layers. This will have an effect and it is important to realise that hyaluronic acid plays a role in the epidermal layers, but it will not each the levels to maintain collagen Type III
No needles solution
Hyaluronic acid is a linear chain made up of repeating sugar units of glucuronic acid and acetyl glucosamine. It is secreted by fibroblasts into the extra cellular space where it helps maintain skin structure and the elastic properties of skin. Hyaluronic acid is a highly flexible hydrophilic molecule with a great capacity to retain water. For many years it has been thought that hyaluronic acid was found only in the dermis. Now it has also been found in the epidermis where it plays an important role in modulating cell differentiation. It is found in high concentrations in the spaces between these cells and has a lifespan of less than 24 hours.
The dermis cells synthesise more hyaluronic acid than they use up and links many extra cellular components due to its long chain structure. However enzymes and free radicals break the structure into smaller pieces and hyaluronic acid molecules have a life span in the dermis of a week or so;

The net result is that applying in the morning and the evening leads to a reduction in fine line and wrinkle depth without the use of needles. The key aim is to use for 4 to 5 weeks until you see the visible results and then reduce the dose to a maintenance dose. One is warned not to use it every day as the plumping effect is delayed and overuse might result in ‘tight skin’. Although this depends on ‘how much filling room’ you have (how wrinkled).
